About the Nominee

Nominee type*
Full legal name of the individual, or the official name of the group or organization being nominated.
Optional — provided to the selection committee only if they need to contact the nominee for verification. Not used for public announcements without consent.
Optional — same as above.
Nominee Address
Optional — nominees do not need to reside within city limits to be eligible.
Be specific and focus on actions taken during 2025. Describe what the nominee did, where, and over what period of time. Prior-year contributions that are still producing impact in 2025 are appropriate to mention as context.
Describe outcomes where possible — for example: number of people served, programs created, funds raised, problems solved, or lasting changes to the community. Concrete examples are more persuasive than general statements.
Anything else the selection committee should know — awards received, media coverage, community endorsements, or longer-term impact beyond 2025.
